What is confirmed

She is ill at the start of the game, and Anca supplies both the herbs and the instructions for her medicine.
Getting the medicine wrong, or running out of time, kills her later in the story. The quest gives no sign at the time that anything is wrong.
A retail endings guide lists giving her the correct medicine as a setup step for seeing every ending in one playthrough, which ties her survival to the family the endings are about.

Known risks

Preparing the medicine wrongly during Withering Away — the water must be hot rather than boiling or cold, and the dose three spoonfuls.
Not finishing Withering Away before the Blood Mass. Side quests stay available while it is open, which is how most players are reported to lose her: not by choosing wrongly, but by spending the prologue elsewhere.

The recipe is agreed by two retail sources. A third tabulates the same options with its outcomes out of order, appearing to mark two spoonfuls correct; that reading is inconsistent within its own table and is not followed.
