What is confirmed

He is met during Bad Blood, after the Blood Mass, and talks about the rebellion.
Admitting the thirst or denying it both keep him alive. He then leads Coen to a crag above Laslea and unlocks further conversation.
That route cuts the Smoke and Ashes objective “Return to Laslea and save your family” down to one time segment.
Feeding on him kills him, and the crag route and the extra conversation go with him.

What is not

Listed rather than guessed. These promote out only against the retail build.

What else his survival changes. The segment discount is the only published consequence.
Whether he appears after the prologue.
How much the return to Laslea costs without him — only the discounted figure is published.

Known risks

Giving in to the hunger during either of his conversations kills him.
Arriving in the Craving Blood state removes the choice: the feed option replaces the others rather than sitting beside them.
Going quietly with Brencis's soldiers earlier in Bad Blood jumps the quest past any chance of saving him.

The clearest published example of hunger management costing something concrete: arrive at his conversation Craving Blood and the game makes the choice for you.
